Your Company
provides the following benefits at no cost to you:
Term Life insurance policy in an amount equal
to one year’s salary through Guardian. This policy can
be converted into a portable policy if employment status changes
(i.e., reduction of hours, leaving the company) and you are
no longer eligible for company-sponsored benefits. This policy
is also eligible for Accelerated Benefits - in the event of
a terminal illness with a life expectancy of less than 12
months a portion or all of the benefit is available to the
insured, without restriction or fees.
AD&D
In the event of an accidental death, an additional benefit
equal to your life benefit will be paid.

Short Term Disability
Plan
Benefit 60% of average weekly salary
Your Company provides a short-term income replacement plan
that takes effect on the 1st day following an accident or
on the 8th day following a sickness (including 6 weeks normal
maternity leave). Benefits are paid for up to 26 weeks. After
26 weeks, Long Term Disability applies. Please note: payments
are taxable as income.

Your Company
provides a long-term income replacement plan that takes effect
180 days (26 weeks) after total disability. Total disability
is a sickness or injury that prevents you from performing
in the functions of your own occupation for up to 2 years,
and of any occupation up to the age 65. Please note: payments
are taxable as income.

more than 100 miles from home or in a foreign country. This
program ensures access to health care and manages medical
emergencies, whether trip is for business or pleasure. If
you become ill or have an accident, Assist America pays for
all services including: medical consultation, medical and
legal referrals, hospital admission, emergency evacuation,
emergency prescription assistance, medically supervised repatriation,
transportation of family member to be with affected individual,
child supervision, language assistance. Incurred medical expenses
are the responsibility of your United Healthcare insurance.
